ISMP Medication Safety Self Assessments

The Clinical Excellence Commission and NSW Therapeutic Advisory Group are pleased to provide healthcare organisations with Medication Safety Self Assessment® tools designed to assist hospitals in developing safer medication systems.

The Medication Safety Self Assessment® for Australian Hospitals (MSSA) and Medication Safety Self Assessment for Antithrombotic Therapy in Australian Hospitals (MSSA-AT) were originally developed by the Institute for Safe Medication Practices (http://www.ismp.org). The NSW Therapeutic Advisory Group (NSW TAG), with the support of the NSW Clinical Excellence Commission (CEC) and the permission of ISMP, has adapted the tools for use in Australia as part of the Performance Indicators and Medication Safety (PIMS) Project.

This web-based data entry program will allow hospitals to enter their data online. Using the web-based program hospitals can assess the safety of their medication practices, identify opportunities for improvement, and compare their experience with the aggregate experience of demographically similar hospitals.

Medication use is a complex, multidisciplinary process. In order to understand and improve the process, many characteristics of your hospital’s systems must be assessed from the perspective of various practitioners. The self assessments should therefore be completed by a multidisciplinary team.

We encourage you to implement these self assessment programs as part of your ongoing quality improvement activities.

The MSSA offers a comprehensive, structured process for assessing the safety of hospital medication management processes. The antithrombotic tool contains items that specifically address the use of antithrombotic drugs in the hospital setting.

We encourage all Australian hospitals to participate.
