National Safety and Quality Health Service Standards (NSQHSS) Standard 10 Preventing falls and harm from falls requires all patients admitted to hospital to be screened for falls risk (10.5) and if any falls risk is identified a Falls Assessment is required (10.6).

The CEC has undertaken a process to review falls risk screen tools and has developed a Falls Risk Assessment tool. There has been significant consultation with staff from LHDs and a process of approval through the State Forms Management Committee.

The falls risk screen tool is the Ontario Modified Stratify (Sydney Scoring) and the Falls Assessment tool is the Falls Risk Assessment and Management Plan (FRAMP).

The Falls Screen is to be completed on admission to identify patients falls risk factors. If falls risk have been identified a Falls Risk Assessment and Management Plan (FRAMP) is to be completed and interventions actioned for the falls risk factors that have been identified in the screen. Care planning should be discussed with the patient or family/carers and falls risk interventions communicated at handover.

Preventing Falls in NSW Hospitals CD/DVD.

The aim of this CD/DVD resource is to facilitate staff to discuss common fall events on their wards and take action to prevent them. There are 4, fall case studies of varying severity that are highlighted in this package; they are examples of actual fall events that have occurred in hospital. Following the fall event is the post fall procedures and an alternative approach to prevent the fall. The information is based on the Safety and Quality Council Best Practice Guidelines for Preventing Falls and Harm from Falls in older people, 2005 and actual Fall events recorded in the NSW Health Incident Information Management System (IIMS).

CEC April Falls presentations 2012 by Professor Finbarr Martin

The Clinical Excellence Commission (CEC) arranged special events with visiting Professor Finbarr Martin presenting at a number of forums including presentations to:

Professor Martin is a consultant Physician in General and Geriatric Medicine (Guy's & St Thomas' NHS Foundation Trust, London; Professor of Medical Gerontology, Kings College London; President of the British Geriatrics Society; Chair of the Falls and Fragility Fractures National Audit Program (incorporating the National Hip Fracture Database and Falls and Bone Health audits) (funded by DH/HQIP).
