The 2012 ISMP International Medication Safety Self Assessment® for Oncology has been launched by the Institute for Safe Medication Practices (ISMP) together with ISMP-Canada and the International Society of Oncology Pharmacy Practitioners (ISOPP).

This tool follows the format and principles of the ISMP Medication Safety Self Assessment (MSSA) adapted for Australian use by Clinical Excellence Commission (CEC) and NSW Therapeutic Advisory Group (NSW TAG). Development of this oncology specific tool was supported by grants from Clinical Excellence Commission and Cancer Institute NSW.


2012 ISMP International Medication Safety Self Assessment for Oncology

The release of the 2012 ISMP International Medication Safety Self Assessment® for Oncology provides an opportunity for Australian facilities to look in depth at their management of the high risk medicines used in cancer care. Participation in the 2012 ISMP International Medication Safety Self Assessment® for Oncology will assist facilities in meeting the National Safety and Quality Health Service Standards which require action to address issues with high-risk medicines and to perform comprehensive risk assessment of medication management processes.

Data for the self assessment can be submitted online through June 29, 2012. At the completion of the project, respondents will be able to compare their completely confidential results with aggregate data from other demographically similar organisations and use that information in their own safety and quality improvement efforts.

Program Overview

Background

The Medication Safety Self Assessment® for Australian Hospitals (MSSA) is designed to:

Heighten awareness of the distinguishing characteristics of a safe hospital medication system; and create a baseline of Australian hospital efforts to enhance medication safety.


Medication Safety Self Assessment

The first phase of the PIMS project involved adaptation for Australian use of the Medication Safety Self Assessment® for Australian Hospitals and the Medication Safety Self Assessment® for Australian Hospitals and the Medication Safety Self Assessment® for Antithrombotic Therapy in Australian Hospitals, originally developed by the Institute for Safe Medication Practices (ISMP) in the US.

These self assessments are risk assessment tools specifically designed to help hospitals take a proactive and system-based approach to medication safety. They will assist hospitals in meeting their obligations for accreditation and can drive change by identifying, and measuring, areas for improvement.

The tools allow self assessment of the medication safety practices within a hospital, identification of improvement opportunities and comparison with the aggregate experience of demographically similar hospitals.
