Be a voice for safety

The delivery of safe and reliable health care in complex clinical settings is reliant on the insight and experiences gained from health care staff and their voice is crucial.

All staff need psychologically safe environments to provide them with the best experience of working in the system and can affect improved patient outcomes.

Diagnostic error refers to a diagnosis that is missed, incorrect or delayed as detected by subsequent definitive information.
