NSW Health observation charts

Calling criteria for early recognition of the deteriorating patient was standardised across NSW through extensive widespread consultation with clinical experts. Information obtained from current research, together with the application of "human factor" principles provides the basis for the Yellow Zone (Clinical Review) and Red Zone (Rapid Response) criteria and the overall design of the charts.

The Red and Yellow Zone criteria are used as triggers to activate the facility's Clinical Emergency Response System (CERS), which mobilises clinicians to assess, treat and review patients who are deteriorating, or are at risk of doing so. The charts are "track-and-trigger" tools, colour-coded with the Clinical Review and Rapid Response calling criteria.

Standard observation charts are available in both paper and digital form for adult, paediatric, maternity services and emergency departments.

The CEC and eHealth NSW have worked in close partnership with Cerner Corporation and multidisciplinary working groups to develop electronic versions of the charts for NSW public hospitals using the Cerner electronic medical record (eMR). In health facilities that already utilise eMR for observations, the paper-based forms should be utilised in procedures during downtime.

The NSW Health Observation Charts are available for purchase in printed form. NSW Health staff can order and print forms via Finsbury Green and more information is available on the NSW Health sharepoint site.

The CEC can provide permission to non-NSW or private health organisations to use the NSW Health Observation Charts under copyright. All requests require completion of a formal copyright request form.
